Welcome to Essential Labs LLC. We are a mobile phlebotomy service proudly serving Pinellas County, Florida. By scheduling an appointment with us, you agree to the following policies and guidelines.

ABOUT OUR SERVICES

Essential Labs LLC provides professional mobile specimen collection services, including:

• Routine blood draws
• STAT blood draws
• DNA specimen collection
• Special laboratory collections as ordered by a licensed provider

We travel to homes, offices, assisted living facilities, and approved locations within Pinellas County.
We do not diagnose, treat, or interpret laboratory results. All testing is performed and processed by third-party laboratories.

LAB ORDER REQUIREMENT

A valid laboratory order from a licensed healthcare provider is required prior to collection for all blood draw services unless otherwise permitted by law (such as certain DNA collections).

Clients are responsible for ensuring their order is correct and includes all required test information.

SERVICE AREA

We provide mobile services within Pinellas County, Florida.
Appointments outside of this area may require special approval and additional travel fees.

PAYMENT POLICY

Essential Labs LLC is a self-pay service.

• Payment is due at the time of service.
• We accept: [List payment methods – Cash, Card, Zelle, etc.]
• Our mobile service fee covers specimen collection and transportation only.
• Laboratory testing fees are separate and billed by the processing laboratory (if applicable).

All completed services are non-refundable.

CANCELLATION & NO-SHOW POLICY

We require at least 24 hours’ notice for cancellations or rescheduling.

• Cancellations made less than 24 hours before the appointment may incur a cancellation fee.
• No-shows will be charged the full service fee.
• If we arrive and the client is unprepared (missing lab order, no ID, unsafe environment, fasting not followed).

CLIENT RESPONSIBILITIES

To ensure a successful appointment, clients must:

• Provide a valid photo ID
• Provide a valid lab order (if required)
• Follow all fasting or preparation instructions from their provider
• Secure pets during the visit
• Provide a clean, safe, well-lit area for the blood draw

Essential Labs LLC reserves the right to refuse service if conditions are unsafe or inappropriate.

SAFETY & INFECTION CONTROL

We follow OSHA safety standards and infection control protocols, including:

• Sterile, single-use needles
• Proper PPE usage
• Biohazard-safe specimen handling
• Approved sharps disposal

Your safety and our staff’s safety are our top priorities.

CONFIDENTIALITY & HIPAA COMPLIANCE

Essential Labs LLC complies with applicable federal and Florida privacy regulations, including HIPAA.

All personal and health information is kept confidential and shared only with authorized laboratories and ordering providers as required for processing.

LIMITATION OF LIABILITY

Essential Labs LLC is not responsible for:

• Laboratory processing delays or errors
• Rejected specimens due to laboratory requirements
• Physician interpretation of results
• Client failure to follow preparation instructions

In the event of a medical emergency, call 911 immediately.

MINORS

A parent or legal guardian must be present for all clients under 18 years of age. Valid identification and consent are required.​

3. Essential Labs LLC operates as a self-pay service.

5. 24-hour cancellation notice is required.

6. Essential Labs LLC reserves the right to refuse service due to safety concerns.

7. Clients are responsible for providing accurate information.

8. In case of emergency, call 911. We do not provide emergency medical treatment.

9. Florida law governs these terms.

 

Failure to comply with these terms may result in refusal of service.
